Preparing for Your Child’s Physical
To make the most of your child’s physical exam, please bring the necessary forms provided by the school, sports team, or camp. In Illinois and Indiana, the Departments of Health mandate medical documentation of a physical exam from the previous year for students enrolling in kindergarten through senior year of high school (K-12).
For children aged 6 or younger, coming in for a school physical, bring their vaccination records and any other relevant blood test results (e.g., lead and hemoglobin). This helps ensure efficient and high-quality care for your child.
What to Expect During the Physical Exam
We prioritize your child’s health and well-being. During a physical exam, our primary goals are to assess their overall health and detect any early signs of illness or disease. We also offer additional screenings for lead, anemia, and tuberculosis (may incur extra costs). Our medical staff will tailor the examination based on your child’s age, gender, and the purpose of the physical. While specific requirements may vary, a typical physical exam includes the following:
- Gathering family and personal medical history.
- Measurements of height and weight.
- Evaluation of flexibility, pulse, posture, joints, and more.
- Vital sign measurements, such as blood pressure, body temperature, and heart rate.
- Assessment of body systems, including the lungs, heart, eyes, nose, and ears.
Our medical provider may also ask about alcohol and tobacco consumption, dietary supplement usage, recent immunizations, and sleep habits. For children under 18, parents or legal guardians must be present in the examination room.
