- Patients with Insurance: Bring your insurance card and a valid ID. We bill insurance directly. The co-pay depends on your plan. If you have secondary insurance, ensure you bring those details too.
- Accepted Insurance: We have a comprehensive list of insurances we accept, including specific Medicaid plans and workers comp insurances. View our full accepted insurance list. If unsure, call your insurance provider or contact us. HMO Plan holders should verify with their insurers.

This Midwest is so cute & the staff is always pleasant. The doctor that has been taking care of me is excellent, she has prescribed everything to make me feel better. I recommend this clinic. Also prior to coming in you should take advantage & use the website for check in/upload you personal information (IDs Insurance cards) for easier transaction once you arrive. I hope you get the same experience as I did or better it’s always been super great for me.

I really appreciate the front desk receptionist, patient care tech, and nurse practitioner. All were very helpful, kind, and calm given how busy the clinic is daily. You can see they love their career of helping and healing patients.
